What is it?

A pilonidal cyst is a pocket that forms under the skin near the top of the buttocks, in the crease between the cheeks. It often contains hair, skin, and other debris. Pilonidal cysts can become infected and form painful abscesses. They are more common in young adults and people who sit for long periods.

Signs & Symptoms

A pilonidal cyst can cause:

  • Pain or tenderness near the top of the buttocks crease
  • Swelling or a lump in the area
  • Redness or warmth around the lump
  • Drainage of pus or blood, sometimes with a foul odor
  • Fever if infection is present

Diagnosis

At Sonoran Surgical, our sugeons diagnose diagnoses pilonidal cysts with:

  • Medical history and physical exam, checking for swelling, redness, or drainage in the sacral area
  • No imaging is usually needed unless there is concern for a deeper infection or abscess

Treatment

Treatment depends on whether the cyst is infected:

  • If not infected: observation and good hygiene may be recommended
  • If infected: antibiotics may help, but most need drainage
  • Minor office procedure: incision and drainage of the abscess to relieve pain and infection
  • Surgery: removal of the cyst and sinus tracts to prevent recurrence. This can be done with different techniquincluding minimally invasive methods, depending on severity
